for many schools, an email is sufficient. you can call and tell them you wanna withdraw and ask what would be preferred? Without even calling, I just emailed and ended it with "please let me know if you need anything else from me." I called to confirm that they got the email and they said everything was fine. but yeah, that format is fine.
Dear X,
Thank you for the opportunity to be interviewed and accepted at XYZ SOM. I have decided that XYZ SOM is not the school I would like to attend and have decided to withdraw from your acceptance. Again, thank you for the opportunity to see your facilities, meet your faculty, and students.
Sincerely,
John Doe
PS I honestly think you should write something short and sweet... just remember the rejection letters from SOM are 3 sentences long 👍

Octodoc That thought crossed my mind, too, but then I realized: Why be "smarmy" to the schools that actually wanted you?
Seems you should send that kind of letter to the ones who rejected you, as in "look what you missed."

Ummmm......to get them to be more respectful in their rejections to future applicants? Yeah, that's the ticket!

You do it to take your bitterness out on a school where it can have an effect. I think a good format would be as follows:
Dear xyx:
I have finished reviewing my offers of acceptance. Yet again, there were far more qualified schools than could be offered a place in my life.
I wish you the best in your goal of training future physicians.
Sincerely,
abc

Hahah.
That's a good letter. It sounds very similar to my rejection letters.Make it sound like a true rejection letter...
Dear xyz,
First, let me take the time to let you know that this was a very competetive cycle. I interviewed at many top facilities throughout the country. I was accepted at all of them. Unfortunately I cannot accept all of the spots. I may be a perfect genius, but that cannot will me to be in more than one place at once. You ended up falling short on my list of schools to attend. Please try again with other applicants next year. Thanks for your time. Good luck to you next cycle.
F*** off,
