Sorry if this is frequently asked, but searching "scholarships" in this forum has proven fruitless. Do you guys included merit-based scholarships from undergrad in the honors category?

Yes, if they are based on merit in the college years, rather than being high school-based.

So if we received a 4 year scholarship, but it was based on high school endeavors, do not include it?
 
In general, HS activities aren't going to add to one's candidacy as a med school applicant. If the scholarship had an ongoing GPA, athletic, or service requirement to keep it active for its last three years, then those merit awards would be college -based. An exception to the general rule might be something along the lines of winning an Olympic medal if it came with college funding, in other words, something so highly memorable it would stick in an adcomm's mind.

If there is a GPA requirement to "renew" the merit-based over a four year period, should the required GPA be stated in the description, or would it be implied that it is college-based instead of solely high-school based? In my case, the scholarship will be combined into a single activity along with Dean's List, honor societies, etc.

I think it's a good idea to state the requirements of a renewable merit scholarship.
