Worsening dizziness in 40 year old man - Possible heart failure case?

This is a case for one of my stories, a true case. A 40 year old man has had dizziness triggered by exertion for at least the past month. A few weeks ago, he started getting frequent falls, and just a few days ago, he started falling unconscious for a few minutes during his falls. Now, he is in the emergency room for Unconsciousness after a fall with possible fracture. He just got his full body X ray done, and he has a lot of fractures, most of them being old fractures that healed right but 2 of them are new, 1 in the left arm and another in the right leg. Blood has been drawn to do a BMP, CBC, LFTs, and Cardiac enzymes. Calcium comes back as 7 meq/L, but no other abnormalities show up on the BMP and a CBC comes back with no abnormalities except a hemoglobin of 8 and a low platelet count. Neurological tests come back normal. Still waiting on the LFTs and Cardiac enzymes. Patient hasn't produced enough urine for a urinalysis, suggesting dehydration. The patient has no signs of infection like a fever and the fractures haven't penetrated the skin.

What is the differential for the patient at this point?
