would my history professor know to include a letter head and signature in LOR?

They should know, especially if they've written LORs before, even official letters from her would go out this way. it's standard to include school letterhead and sign.but if it would make you feel better, you could always send her a quick email to remind her, saying you're under a lot of stress and you've been reading too many horror stories on SDN lol
 
And I actually sent the link on how to write an AMCAS LOR to my professors haha...but they were pharmacy professors so I didn't want to take any chances. So I get where you're coming from!
 
I'm requesting letters early (through Interfolio) for an application next year, and I've been reminding everyone that official letterhead and signed letter are required - from people who I know have never written a med school LOR to science professors who definitely have to people who are faculty/part of medical school administrations. Take no chances!
