Writing a Cover Letter + Resume for Professor

Hi guys, I've recently asked my organic chemistry professor to write me a LOR but he asked me to first submit to him a resume + cover letter. I have finished writing my resume but am unsure as to how to write a cover letter for him. Most of the templates online deal with job applications. Can you guys write a template for me to follow? Thanks
 
Make it a mini-mini personal statement. Briefly state why you're applying and why you think you're awesome. Feel free to highlight key points you think make you a good applicant -- things that will make it easier for he/she to write. Just a little glimpse into your life at the moment.

you may want to include why you have asked him to write the LOR. What you enjoyed about his class, him as a teacher etc.
 
I would honestly just give him a personal statement. Put it in letter format if you have to (easily found on google or Word templates).

Yup. If you have your statement done or a presentable draft, throw it in there. Even with a transcript, DAT scores, resume, and a personal statement draft, I still included a cover letter. There was plenty of info there if they felt like sitting down and reading, but if they needed to crank it out for whatever reason, it gave them some quick info at just a glance to hopefully avoid something generic from happening. And you can add a sentence about your gratitude to them.
 
How should I format this letter then? Like what should I write about in paragraph 1 or 2? Or should I just shotgun it? lol
