well it is not on my transcript, but if they request my "mess-up" report it will be on there. do i play the game "if i put it they will look it up and just nix me because i am a weak applicant anyway" or the "if i dont put it they might not request my **** up report and up my chances, but then if they look i have stuff on my report" this is a slippery slope for a weak applicant.

gpa is 3.2 but i have 100's of shadowing hours , i am a full time dental intern/assistant, and some other stuff but that gpa is horrible and i dont have biochem. dat is solid enough. im applying to 1500 bucks worth of school so that should help but im really hung up on this last notion of clicking "yes" or not. what to do, what to do?

If you don't put it on the application and they do find out [which they will - they check up on that sort of stuff for a reason] then you will have a 0% chance of being accepted.

You're better off putting it on there and seeing what happens. They may not look down on it as much as you think, but they will definitely look down on you lying on your application.
