When you inherit a patient from either a retiring colleague or from another practice (e.g., due to insurance issues), do you dig through all the prior imaging, pathology, and chemo records yourself to confirm accuracy of prior documentation, or can the records from the previous physician be relied upon for accuracy? We see many of these patients in our program, and re-inventing the wheel in this respect would take hours for the average patient.

You decide. If the documentation that comes with them makes sense, I take it at face value and confirm with the patient when I meet them.

But most people’s documentation sucks goat nuts, so I often spend a little time reinventing the wheel.
