Near identical question on both UWORLD and AMBOSS. Uworld has been saying ultrasound, amboss has been saying surgery. Did the guidelines change? I always thought you are supposed to do ultrasound first.
I think for board exam purposes they have to tell you specific history and exam findings like sudden onset, atraumatic, with high riding testicle. In that case you're supposed to know its torsion.
In reality ED will order ultrasound --> rads sees absent/asymmetric reduced flow in the pained side --> surgery
