Here's what I've gathered. Please post prompts for schools that we're missing. *And Columbia has recently changed to AMCAS, so their new prompts would be much appreciated.

UWSOM

AUTOBIOGRAPHY: You must submit a brief autobiographical statement. It should describe the origin and development of your motivation to be a physician and any other issues of importance. The Personal Comments section of the AMCAS application may be used to satisfy this requirement, or an additional autobiography may be submitted with your supplemental materials. Please write your name and SSN at the top of the page and indicate on the data form if you are submitting an additional autobiography. Due to the volume of applications we receive, publications or manuscripts will not be included in your application file, and cannot be returned if sent. Instead, please submit abstracts or other brief descriptions of your work.

"WHY UWSOM" STATEMENT: You must submit a statement, not exceeding 200 words, about why you want to attend the UWSOM. This cannot be fulfilled by the AMCAS Personal Statement or the additional autobiography.

CCOM

I don't remember the exact prompts, but basically there was one essay on why CCOM and why D.O. You also have to submit a resume with experiences.


Loyola

1. What do you consider a valuable experience in your personal development? This might be a decision you have made, an achievement of which you are particularly proud, or a person who has influenced your life.

2. Describe briefly how you have dealt with a personal or academic problem you have encountered.

The University of Colorado Admissions Committee is interested in obtaining additional information from applicants who have lived in a rural or a medically underserved area. A rural area is defined as a community with a population of 15,000 or less. A medically underserved area refers to an urban or rural area without adequate health care services.

Is there anything else you'd like us to know about you? (my paraphrasing)

Strong motivation for a career in medicine, human compassion, an abiding interest in the problems of people, leadership skills, the ability to communicate ideas effectively, and a high level of personal integrity are essential qualities of the physician. Additional criteria also include: socioeconomic background, first person in immediate family to graduate from college, multilingual proficiency, responsibilities while attending secondary and/or undergraduate schools, community involvement and geographic diversity.

This was a box about 1/3 the size of the personal statement.

They had like 10 prompts, but each were real short, just a few sentences. And their instructions said something mysterious like "you don't have to answer all of the questions, just the ones that will help us get to know you better":

2 page autobiography, I didn't know it was coming and was totally unprepared to discuss my early childhood, worth thinking about it early.

2. Then they give you a list of possible career paths (like Surgeon general, academic medicine, director of MSF, etc.) and then ask you to say which you're planning to pursue or something silly.

UCSF

Best secondary ever! They basically want $60, your signature, and you can write them an update if you want

UC Davis

Most brutal secondary I got. Tell us about everything you've ever done and why it will help you be a better doctor. Really, there's nothing you can do to prepare for this one, it's ugly.

definitely the easiest--send them a check and they are happy. if you spent time doing something between undergrad and application you need to submit a letter explaining why/what you did with your time. if you do not have a committee letter (and instead have individual LORs) you have to write something explaining why.

Columbia
who knows what they will do this year now that they've switched to AMCAS, but here are the questions they used last year:

also an easy one, though the table of courses is annoying to fill out after you've already done AMCAS. i wanted to attach the doc here, but it is too large. PM me if you want a copy and i'll email it to you. otherwise the two main questions are as follows:

very easy secondary. they want a photo and 95 bucks.
if you've got some special reason for wanting to go to tufts they ask you to explain it in a paragraph or less. they also ask you to list any family members who have gone to tufts. i have this pdf if you want it.

if i recall, there wasn't much room to answer these (they do mean brief when they say it) and i had to chop down my "most important experience" quite a lot. like many schools, i had to include a letter explaining my lack of composite LOR.

Jefferson really requires SAT scores? 😱 That's almost enough to make me not want to apply there. That was so long ago, and back then I had this attitude that you don't study or prepare for standardized tests...meaning I just took it cold, and sure I did okay but it's not in any way representative of who I am!!!!! This is really making me mad 😡 👎

edit: I also don't even remember my SAT score. Really. I think I blocked it from my memory!

Wake requires SAT scores as well. I don't think they check though because I forgot my subscores so I just made them equal what I knew my overall score was.

Many schools ask for the same stuff as the AMCAS application because many schools are going to a pretty neat new system of not revealing the numbers to the interview folks. Once you submit your AMCAS, the members of the adcom or the dean that does the review will decide whom to interview. Once you have been selected for an interview, many schools are now only forwarding the secondaries and not the AMCAS application. This way, the interviewers are not biased by exceptionally high or low numbers, but look at an applicant as a whole, and through the interaction they have during the interview, not just as a number.

Its annoying, it feels redundant, and honestly, when you've worked all day and have life happening at the same time, nobody exactly wants to come home and try to be positive and insightful AND creative, but I assure you it is well worth it. When it comes time to pick whom the schools want to meet in person, everything you have said about yourself will come into play and everything you write about yourself gives the admissions committee its overall impression.

uh... i guess that's a better, more happily deluded thing to believe than the truth- that AMCAS is just another hoop to jump through before getting to the school's application, which in their narcissistic mind is the only one that REALLY matters. Welcome to the land of ego, kids, and get ready for a long road of more ahead- you're not in Kansas any more. Oh, and prepare to drop lots of c$sh into the evil monkeys' hands...

Most people will discover at some point in this process that once you've written the first few secondary essays, the majority of the work will be a cut and paste job. There are a few standard themes that they will be prompting and once you realise that you can adjust your essay to 'fit' whatever they're specifically asking, I'd say go forth and recycle! I wrote about 5 essays that I was able to edit and tailor specifically to each secondary. 👍

I didn't know what some of the abbreviations stood for. Schools that were already listed under the state or city name (e.g. Miami) I put in that order (i.e. I didn't add official names like "University of...").

We're still missing a bunch. I took off some that didn't have any info, particularly HMS, which people will probably be interested in. If you have any to add, post them 🙂

I don't think they care that much. It's not as if we're seeing answers to a test or something. The essays they receive will be of a higher caliber so I don't see what the problem would be.

If you post new prompts, I can add them to the old posts. Unless a web page is really wanted, in which case I could do it. But I sort of feel like that's overkill. It was already a lot of work for LadyBulldog to collect all the prompts from last year's thread.

Q, as far as changing the prompts, I think they're more interested in knowing about us as potential students, so having a head start on the prompts shouldn't make a difference (hopefully).

Thanks! I added it to the list.

As for the second one, it's tough. Personally I might discuss how I'm an Orthodox Jew, and it is very unusual for Orthodox Jewish women to go into medicine, so I would definitely contribute to diversity. If you've had any sort of unusual educational experience, or international experience, that would probably be good to talk about. But I'm sure it wouldn't hurt to leave it blank.

it seems that temple is just "why do u want to go to temple....." and they give you like 3/4 to 1 page to write about it 👍

most are...some are from the year before (like columbia) but they apparently don't change that much

So most of these questions are about the applicant's acheivements. I heard that they sometimes ask ethical questions too. Is this true?

I never had an ethical question on a secondary. They'll save those for your interview.
 
For 2005, Case had an essay on any topic or activity that was important to you, and it could not be related to medicine or science. As I recall, you had a LOT of space compared to other essays, but I can't recall how much. This is the one where you should be REALLY glad that you have a non-medical hobby... if not, ride your bike around the block a few times starting right now so you can write about Lance Armstrong.

I can just imagine the instructions:

"Please write a 3-page essay on any topic or activity that is important to you and not related to medicine or science. Note: for the next 7-10 years of your life, you will have virtually no time to pursue this topic or activity, and if you do, you will feel guilty about it."
 
U of Missouri doesn't require an essay unless you're a reapplicant. I've talked to the admissions office. If you are reapplying, this essay is very valuable. You need to state something to the effect of:

What have you done to make yourself a better candidate since you last applied to U. of Missouri?

The admissions office told me that if they don't see improvement here, you won't get an interview, when they like what they see, the interview stats for reapplicants are higher than the normal admissions cycle.

It depends on the secondary. I heard that Duke's secondary is pretty long, with a few questions on ethics.

So I am gearing up for that
