Break down your volunteering hours again: volunteer builder, mentoring.

What is your purpose as a physician? How should I interpret your advocacy in this context (sustainability, gender rights, anything else)? Most of your clinical experience seems centered on just emergency medicine.

My purpose in my personal statement was basically about how i enjoyed serving patients and seeing the impact i've made so far in clinical settings and i want to expand my scope of practice to the fullest and be able to freely practice medicine. I also addressed my wish to work with immigrant communities and non-english speaking populations.

separate from my personal statement, a large part of my application is also about teaching/mentoring which within my 2300 hours i've spent 500 hours precepting EMT students. i have not spent much work directly serving the lgbt community.

• 350 hours for a pro-bono design-build club (i was originally studying architecture my freshman year) we design and build small projects for local communities and groups

• 100 hours with a educational non-profit serving low-income kids of color which i am an alum of and have worked with for over 10 years, i'm gonna continue mentoring this summer and offer MCAT and EMT tutoring

• 50 hours as a volunteer EMT

yes all of my clinical experience as well as my shadowing is in acute care settings (pre-hospital, ED, and ICU) is that an issue? i did bring up in my shadowing section i wish to shadow in clinic with a PCP and inpatient with an internist to diversify the specialties i have seen.
 
Service orientation activities include food distribution, shelter volunteer, job/tax preparation, legal support, transportation services, and housing rehabilitation. It appears you have zero such activities, and it's unclear that the design-build club satisfies this category (more like teaching/mentoring). Consequently, your application may be at risk of getting screened out at most programs where you do not demonstrate a very strong mission fit with your purpose.

I don't think it's a big deal to have the bulk of your shadowing and clinical experience in acute care so far. Your approach to observe hospital/internal med is reasonable.
