I'm torn because I'm a TX resident applying to mostly TX schools, and I've heard TX schools are weighing Casper more heavily now. I really don't want to take a gap year but I will if necessary. For context I have a 3.95/524 and would be happy with any TX MD school.

They use it as a part of your application and don’t “weigh it” more heavily than in the past.
Some schools look at it early and some not until after they interview you.
If you are otherwise ready in terms of volunteering and healthcare related activities this score would not be a reason to do a gap year
