It says (i.e, family member, closed friends, etc) besides the question, though.
So, I went for "no". I hope it's okay.

I think you are supposed to put "no" unless you know the person well. If it's purely a shadowing situation with limited personal involvement, then definitely say no. But, if it is someone you have developed a relationship with, you can say "yes." I put "no" on all of my secondaries for that question.

Acquainted doesn't necessarily mean relative or close friend. As I recall, one of the secondaries that asked me that was actually trying to ask me who wrote my LOR because it asked for a name, address, and phone number of the person I was acquainted with, but didn't ask for this information about a DO anywhere else. Also, it was in the same section as other LOR's information.

I was filling out for NSU and it is asking the date and the institution that the DO was graduated. So, I just put "no".

Just because they ask it doesn't mean you have to know it. Besides, you could just ask when you meet a DO for LOR purposes. None of the schools I applied to asked, but if they did, I would find out. I don't even know the date that MY doctor graduated or where he graduated from, but he is a DO and I am acquainted with him. I know where my mentor went, and where he did his residency, but I don't know exactly when that was.
