What is AAMC referring to here?

Like using a base to deprotonate the alpha-hydrogen and then attacking an alkyl halide?

I think they are referring to a nucleophile attacking the carbonyl carbon. Not sure though since I didn't follow the AAMC guideline, just trusted TBR.
 
maybe changing the carboxylic acid into another carbonyl (such as to a alcohol) via a reaction?

Somewhat off topic but did you also trust TBR with regard to what mechanisms to memorize (as opposed to just knowing the reagent/product)?

IMHO, the idea of TBR is not to memorize per say. They introduce tons of mechanisms so you are familiar and not surprised when you encounter it. I made notes for every mechanism thinking I would be going over it to memorize. I have not. So far I have taken a few AAMC's and have been scoring high 30's with no problem in o-chem. Don't fret about memorizing. Work through the passages. Understand mistakes. Build foundational concepts. You got this.

You are right about this. Everytime I do TBR passages, I get around half of them wrong, it makes me feel like I have to memorize all the reactions. I try to fight this feeling as much as I can. However, the AAMC passages are more about seeing the patterns of reactions and trends.

Which you do SOOOO much better because TBR is a dime piece when it comes to synthesizing content review and critical thinking
