I would spell it out the first time you said it at least

Personally, I would strive to use words or phrases that didn't require abbreviation. If you're abbreviating, it's always good practice to define the acronym or abbreviation before you use it. For example, You could say "blah blah gave them Comfort Measures Only (CMO)" then in the next sentence "However, when they discussed it with Dr. Throwaway, they decided that CMO was not the right call" or something.

Ditto. Writers should NOT assume that every reader will be knowledgeable with medical shorthand.

Agree with Goro. I've seen medical school faculty with degrees in anthropology, English, library science, public health, sociology, as well as natural sciences (histology, immunology) on the adcom over the years. None of them may be familiar with an acronym such as CMO. Always spell it out the first time and put the acronym in parenthesis.
