I took courses in coronal polishing course and a dental x-ray radiation. Is it appropriate to put them under the Academic Enrichment section or dental experience sections?

I would put that under dental experience, as they are classes specifically related to dentistry.

And do you think, I should also mention a UCLA predental toothwaxing course in the dentistry experience too? Because in another thread someone else suggested that it go in the Academic Enrichment Section?

But how do you fill in the "Name of supervisor" "Position Title", etc?

I put all of those, including the UCLA class, under the Academic Enrichment section, or whatever it was called two years ago... man, seems so long ago already, haha.
