All schools do thorough background checks on all matriculants. I can assure you that you will get in bigger trouble then than if you were honest about it in the beggining. Of course if you have nothing to hide, there's nothing to worry about.

I was wondering if any schools you applied to made you sign a form that allows them to check your academic honesty record when applying to dental schools?

I remember reading here on SDN that Temple Dental has a secondary question where they ask you to grant them authority to discuss your application with anyone (see this thread). Pretty sure that will let them inquire about your academic honesty record.

Is the purpose of the schools requires the letter about the disciplinary history to determine if you told the truth on your aadsas application? If you say you did have a disciplinary record on the aadsas and they confirm it with the letter from the dean of the school, are you in a good position?
Having any disciplinary action against you is never good. You're in a better place than if they randomly found out about it as opposed to you being honest.
