hi!

can anyone please explain to me what ( A,B,C) grading system means,
regarding the preliminary test.
is < C > equivalent to 50 % of the mcq's, or more.

and do they delete one mcq for every wrong answer??

# Patient has apical abscess in 46 and needs extraction. Already 4.4 ml. of 2% lignocaine with adrenaline 1: 80.000 is injected, but when you try to extract tooth it is still painful. Discuss the possible management options.

4.4ml of 2% lido might not be enough, considering the inflamation process going on. The following options might be considered:
1) Additional block using 3% Mepivocaine (it's action is synergic with lidocain)
2) Long Buccal nerve block
3) Additional infiltration, both bucally and lingually


# 65 year old female patient on oral biphosphanates. She has several grossly carious posterior teeth. how to manage?

Bisphosphonates may alter bone homeostasis sufficiently that the ability of bone to heal after minor insults is compromised. The bone may also become secondarily infected and sequestrate, leading to a lesion that appears clinically similar to osteoradionecrosis. Therefore, if there's an option to avoid surgical procedure, those posterior teeth should be managed conservatively, including endo. If there is no choice but to extract them, perform extractions and other surgery using minimal bone manipulation with appropriate local and systemic antibiotics, using mouth rinses to reduce the microbial load; follow up to assure healing.

i think i can add a little bit based on my personal experience
it is always beneficial to put a patient with abcess under a course of antibiotics prior to extraction
blocks in this situation becomes difficult as the the environment around the abcess is acidic which nutralize the alkaline nature of lignocaine thus LA doesnt work .well in that case a local infiltation such as intrapulpal,interdental will be beneficial apart from from the usual block.
buccal and lingual infiltrations are mostly ineffective in this cases becuse of the thick cortical plate
last but not the least prayers

I have to disagree a bit ... the acidic environment near the abcess shouldn't effect the lidocaine potency at the site of injection during the block (which is about 7-8 centimeters away). Anyhow ... 4.4ml of anesthetics are not plenty, since statistically, in a perfect setup experienced dentists successfully perform IDN block only in 80% of the cases at the first shot. The dentist can surely add mepivacaine tithis block, and the infiltration would not numb the tooth, but the surrounding tissues.

was out of action 4 few days ... sorry anyways my soln to abcess case quite agees with somali :

1 my approach would be :

a i/d if any visible swelling otherwise opening on pulpal floor for exudate to escape and hence the stress will reduce this wil first give immediate relief from pain which should be the first aim of dentist, postpone the extrn and antibiotic coverage and then extrn at next visit ... pl comment 😕

Guys I've been through the 1000MCQ bank and found some 50 questions I found to be a little confusing. If you have 30 minutes of your day just to double check them and which ones need to be corrected … I'd be really grateful!

The problem is that the dentist has already begun the extraction procedure ... you can't just pull out in the middle of it and tell the patient to come back in a week after antibiotics course. I think we're expected to answer about the ways to anesthetize the region and successfully finish the extraction

in this case i feel they want to test our ability to handle a situation and i feel we can keep the antibiotic therapy as a option if the patient disagree then we can proceed after a through explanation of the situation and possible complication.
it is also said that in a abcess prior to antibiotic, extraction can actually help to spread the pus in areas of low resistance and into the circulation
however in extreme emergency we can even attemp general anaesthesia

I really don't know to write about my prepartion for the Prelim exam
Everything was mesterious for me I did not know about this forum or I did not have about what you are talking about Cawson Dental decks or 1000bank questios
Only I had the ADC paper of prevois questuions they send it to me when they accept me to sit Prelim which contains old and out of dated mcq
All I did is to plan what to read so I took the secctions of the exam and start to concentrate on it like for exmple in pros I think what is important for me as a general dentist to know in this subject clinically

I really depended on Oxford ,Chuchil pocket book of dentistry little from clinical problem solving in dentistry and dental secret, really I benefit from my previous clinical experience and the most important think is that you should think logically in your answer and think that if this was a case came to your clinic how you are going to manage it so do not onlty keep the information by yoiur heart but also try to think

And finally I sit the exam as a try to see what is the scope of the exam andI learnd that it is mostly clinically based.
Remember I am not living in Ausralia and I do not have any contact with any one who passed the exam but I depended on myself

I happened to know this Dental Secrets.
it's a book from Harvard dental school. It's a very good Q&A source for dental knowledge.
smart minds think the same or whatever 😀I bought it last week as well before composita recommeded here and also see what odonta says up there 😀

I was highly impressed finding this group. Thanks to all for you great effort.

I consider that it is so sad that Australia desperately needs so many dentists, and will never be able to cover that need for dentist from university places at Australian universities. The logical solution would be to allow OTD to work under supervision for up to 3 years before sitting the Final ADC exam, like the dentists from Canada, Hong Kong, Malaysia, Singapore, South Africa and the US. If I am not wrong, that arrangement exists in the United Kingdom.😀

Anyway, thank you guys for your efforts creating this group, and I look forward to contribute with this group.

hi there,
i just finished my oet,
it is easy but the trick is to prepare according to what they want.
what i mean is the letter has a particular format that they want so just practice letter writing with this in mind.
then the reading does get a little tricky so learn to scan articles and pick out key words.
and when you get the test read the questions first then the passage, the questions usually coincide with the paragraphs.

Thank you for taking time and checking the MCQ answers with us juniors!!!😳

Reg. MSQs on the test, the point there that questions are really tricky and you need to read them a few times before answering. Take your time and be cool. You have 2 hours for 80 questions, it's like 1,5 minutes for each, so it's alright!!
We who studied many months before the test, read a bounch of books, were trying to solve the msq's puzzle, now we've got a confidence, I am sure that most of us can go through it. But it's true that the test is tough, very tough.

Now to the msq's.

Q 69. B?
Q 153 I think it's E (source Dental Decks Prosthodontics p291 there it's clear that area for support can't be determined by surveying)
Q 292 B
Q 334 C
Q 418 A
Q 421 B is it periapical anyway?
Q 670 A
Q 672 C
Q 731 is it B? But in Boucher p 213 the plaque is "soft film composed manly of bacteria and cannot be rinsed off the teeth"??? see also Q 293
Q 898 and Q 999 are confusing, what is the right answer if anybody know???
Q 981 B (C is related to healthy gingiva)
Q 1028 D??

No, many OTC participants failed, too. But the pass rate is much higher than for those not in it. I am not in OTC.

they are suppose to inform the venue on 31 st aug
i have written them over 20 emails asking for the venue but are not even botherd to reply😡😡
