So, I'm finishing my civi sponsored residency, but continuing on into fellowship. I'll be eligible for the additional specialty pay bonus of 15k starting next month, but do I have to apply for it? If so, does anyone know which form I have to fill out? Thanks for any help.

It's not a form. It's a contract that has to be prepared for you (at least in tbe army)? If you are on active duty, you should be getting a lot more than that. You should be getting:

ASP 15,000 yearly
ISP 20+ K yearly, based on you specialty
VSP of 416/month for your first six years
If you are board certified you'll get 208/month.

Even when I was in the line and deployed, the hospital helped me with this. The line unit S1 was completely clueless. I believe that your location showed Alabama. If you are army, I would give Ft Rucker a try. Someone in their med company S1 shop may be willing to help you. Failing that, I would contact the surgeon general's office. Anyone else have any ideas?

I am a little confused on your status. Are you on active duty participating in a civilian fellowship? (Full time outservice training)

Every year you have to apply for the contract and finding the right people may be the most difficult process of getting the additional pay. Given your unique situation I would contact nearest military GME office or HPSP office in Falls church? to help you. Good luck
 
You have to contact the unit you are currently assigned to. Who do you communicate with regarding issues, such as pay, leave, etc.? Someone at that unit is responsible for drafting the contract. Oftentimes, however, you have to remind them to do so.
 
The special pays for Army officers are done via contract from the unit S1. if you are in a civilian residency or fellowship then you are assigned to the ASD. Your personnel clerk at the unit will initiate your contract. Normally the OPS manager at the company will send you the contract if you are eligible. if you are not listed as eligible, you need to send a copy of your license to the company so they can request that the Surgeon General's office put you on the eligible list for ALL your special pays. if you are assigned to a military hospital (on your orders) then go to the S1 shop.

Speaking as someone who was until recently assigned to AMEDD Student Detachment you may have to remind the Operations Mgr to generate the contract. Despite being eligible (a current copy of my active license was in place at the company) she didn't do it automatically as it had been done at the command where I was prior to ASD. In my previous command a contract was generated for me in April and my specialty pay deposited in August. This past year it was June and I realized that I had not been contacted. When I did contact the OPS Mgr (after having to call the supervisor to ask that she have the OPS Mgr answer the phone) she did have a contract for me within 2 days. I am waiting to see if I receive the pay next month.
