So I’m having an LOR “written” by a doctor I know, but she wants me to prewrite most of it (kinda a good and bad thing I guess).

All my LORs will be sent to my pre-health committee for a committee letter.

Dumb question then - but who do I address the letter to? Should I just keep it simple and say “Dear Sir or Ma’am,”?

I would suggest something like "Dear Admissions Committee". My guess is a doctor probably wouldn't say "sir" or "madam" to fellow doctors.

A pre-health committee isn't an admissions committee.

The doctor will be writing the letter as if it's being sent to med school admissions committees. Often times committees will send their letter on top of the other letters in a "packet." It doesn't make sense to address it to the pre-health committee.

Just write "To whom it may concern" and write it as if you were sending it to an admissions committee.

I must have had a stroke or something that's totally correct, my b

That said my technical writing prof told our class "to whom it may concern" is sort of out of favor but I don't really know how much it matters, doubt it's something to get hung up on.
