I am an intern about to begin my prelim year in a couple of weeks. What advice could the veterans through my way? Specifically, what rotations could I take this year (i have opted for 2 neurology months as of now, but i could change them if i wanted). Secondly, what books should i start gathering for next year?

learn as much medicine as you could because this is your only opportunities. you will need this knowledge to help you to manage your ward patients....unless you want to consult medicine for every medical problems along with their neuro problems. ID, cardiology, and critical care are highly recommended besides your other ward month.

i agree with you in the fact that this is going to be my last chance to work at internal medicine. but before i join neurology, are there some rotations like radiology or ophthalmology that could help me?

You could consider doing neuroradiology, but I would try to get as much medicine related electives during your intern year. You will have time depending on your neurology program to do neurology related electives like neuro-optho and etc.

I don't know if these rotations are available for an prelim medicine program, let alone for an medicine intern. sounds more like transitional programs to me if you want to seek out radiology or ophthalmology experience. You really do need the medicine experience to help you carrying through day to day ward management. There are plenty of elective time during your 3 year neuro residency to explore neuro-radiology or neuro-ophthalmology.
