I would do as many qbanks as possible. The way I would prioritise them UW > all NBMEs > Rx > Kaplan. There's pastest, medbullets and loxolab, which are free but I didn't use them so can't comment. Lecturio's questions aren't worth it.

I wouldn't worry about the retired NBMEs, but I would do all the ones that are available. In my experience, closely reviewing all the NBMEs is very good use of time.

Can I ask what you mean by this? What I do is find out the answers to the ones I get wrong, and figure out my mistakes. Are you suggesting something beyond this?
I would also review the ones you get right. You shouldn't just read the ones you get wrong, you should memorize the fact that you didn't know that made you get the question wrong (i.e. make flashcards)
