I need advice. Applying for PEM fellowship this year and I am 2 years out of residency. Do you think I should get a LOR from Peds Residency Program director?

I don’t know about PEM specifically, but don’t most fellowship programs require one of the letters to be from a program director? Have you checked the fellowship websites?

Absolutely. Most fellowship websites will list it as a requirement.
