So, for a few of my AMCAS ECs I do not have any contact information to give them. For instance, I volunteered at a hospital two years ago, and my contact person there died. Is it a big deal if I don't leave any contact person, and just give the name and city of the hospital?

Try calling the Hospital you volunteered at and get the person's name who replaced them. It's not too big of a deal.

I wouldn't worry about it, esp. if it's just one entry. If you didn't have contact people for any activity, that might be a little fishy. :p
