AMCAS Transcript Form

Since AMCAS is not available until mid may, I won't have access to their form (I have to send in my transcript before that, for various reasons). What is on this form exactly? Wouldn't including a page with 1) My Name and 2) AAMC ID serve the same purpose?

People have sent transcripts in the past without the form and AAMC still received them. I'd just make sure that you include the details such as your name and AAMC ID.
