This is what UIC requires in their supp. application:

Submit the PharmD Program Supplemental Materials directly to the UIC College of Pharmacy at the address below by 5:00 PM on January 15*. The supplemental materials consist of the following pieces.
a. Pre-Pharmacy Coursework Academic Record Form
b. Personal Datasheet
c. Two required Letter of Evaluation Forms (even if letters have been submitted through PharmCAS)
One letter must be from a professor, a TA, or an Academic Adviser.
One letter must be from a work or volunteer supervisor.
d. Each letter must include a signed Right of Access Waiver Form.
MY question is, how do I obtain materials (a) and (b)? Am I supposed to contact the school directly? Or wait for an email? I'm confused.
