This is what UIC requires in their supp. application:

(copy and paste from their website)

Submit the PharmD Program Supplemental Materials directly to the UIC College of Pharmacy at the address below by 5:00 PM on January 15*. The supplemental materials consist of the following pieces.
a. Pre-Pharmacy Coursework Academic Record Form
b. Personal Datasheet
c. Two required Letter of Evaluation Forms (even if letters have been submitted through PharmCAS)
One letter must be from a professor, a TA, or an Academic Adviser.
One letter must be from a work or volunteer supervisor.
d. Each letter must include a signed Right of Access Waiver Form.

MY question is, how do I obtain materials (a) and (b)? Am I supposed to contact the school directly? Or wait for an email? I'm confused.

A,B, and C are all under the supplemental application materials on their webiste. If you click on this link http://www.uic.edu/pharmacy/student_affairs/prospective_students/index.php
you can see that "Supplemental Application Materials" hasn't been made a link yet and that it is coming soon...you just have to wait it out I guess.

So in short, you don't need to wait for an email or contact the school, just wait for the materials to be made available on their website.
 
hmm not sure. i'm applying but haven't looked into that yet. the easiest way is to call the school, i had a couple q's last week and had them answered by the admissions/student affairs peeps when i called.

edit: ah the guy above posted at the same time i did. question resolved!
 
I may apply but I wish they didnt require so much for their supplemental! Are you guys IL residents?
 
The supplementry application will appear in September/October. It doesn't take that long to do.
 
Does it matter if you apply to UIC early or not, considering its not a rolling addmission school?
 
As long as you apply by the deadline, all applications are reviewed at the same time.
