Application for Licensure Question

I was looking at my state's licensure application and saw that I am asked to record my pre-internship hours. There is space to list 3 supervisors and to state how many hours i acquired while working under each one. I have had many different supervisors, so I'm wondering how that works.

For those of you who applied for licensure, did you have to write down how many hours you earned with each supervisor? What do you do if you had more than 3 supervisors?

Also, i don't really recall exactly how many hours i earned with each supervisor...

I believe I got a letter from my DCT stating the total # of hrs completed, supervision hrs, etc. We also had an excel tracking sheet that we filled out daily/weekly. Gotta love the VA!

It's really important to know the state laws about documentation (both internship & post-doc) bc some are quirky.

Thanks so much for your response. I feel confident about tracking hours at internship, but not the pre-internship hours that I did over many years in graduate school. I have the hours (because i still have the record from when i applied to internship) but i don't recall how many hours i earned per supervisor. Did you have to record pre-internship hours for your state? If so, did you have to record all of your supervisors. I also had many supervisors, and i feel as though there is not enough space on the application to list them all.
 
I thought you meant pre-doctoral internship....

The states I've been licensed in just needed a copy of my degree, proof of internship and post-doc, and the state licensure form. ?..and of course a check. I believe EPPP score gets sent in separately.

My state (Maryland) allows you to use your pre-internship hours but I am not sure how to record them. Thanks for your help though.
