I did a quick google search, but could not find the answer:

ARDS causes alveolar collapse, pulmonary edema, and pulmonary infiltrates. This results in less diffusion of oxygen from the alveoli to the bloodstream.

Review books like step-up list ARDS causing decreased 02 because of this difficult in diffusion. These books also list respiratory alkalosis as a result of ARDS, however, and that doesn't make sense to me. I presume this is due to quick breathing in ARDS and the patient blowing off all his CO2. However, if the aforementioned problems prevent 02 from diffusing, why isn't CO2 also affected?

b/c CO2 is perfusion limited whereas oxygen is diffusion limited
