I'm going to make this short and to-the-point: I am in my second year undergrad. I was on academic probation my first semester, took a break, went back to school and did mediocre for 2 semesters (3.1 the first semester back, 3.5 the second semester back). Then last semester I tanked dealing with depression, 2 deaths in my immediate family, and just being distracted. I got academic probation AGAIN.

So I've now had academic probation 2 times. Am I ruined for MD? Am I ruined for MD and DO? I'm so scared. Please any advice.
No, not at all.

1) You have to get your depression under control or med school is a no go. It's a furnace.

2) You have plenty of time for an academic comeback. Rising GPA trends are always good.

3) We like come from behind stories.

No you aren’t. But you’ll need to show improvement. Probably need to consider finding help to in the way of a therapist or counselor to help cope with the stressors in your life in a way that you won’t stumble again in your academics.

The reason I say this: Things in life are going to happen in medical school that aren’t ideal and you won’t be given too many chances to get through before you’re given the boot.

You are still early enough in your undergrad that you can come back from this. Your mental health is #1 though. Get that under control or your grades will likely continue to suffer as they have been. Really gotta buckle down now though. Get that upward trend going and if your GPA is still too low when you graduate you may need a year post-bac, but it can be done.
