Army Pediatric Emergency Med

Does the Army train pediatric EM docs? After a peds residency or after an EM residency?
The Army does have availabilities for peds EM fellowships. They are civilian programs rather than military, meaning the Army will sponsor fellows to complete training on the outside in return for more duty obligation.

it may be listed, but good luck getting approval.

adult EM--> can see adults. can see peds

peds EM--> can see peds.

they may let you do it from EM->Peds EM but the people I have known who have tried to go Peds -> Peds EM have had zero luck. rules are of course made to be broken and if some general's kid wants it they may get it, but do not plan on this being a viable career option unless you have a lot of connections. basically because where are they going to put you afterward? can't put you in an EM billet (who can see kids anyway) and you aren't going to be seeing clinic.

as an aside, during my own gen peds days I was at a MEDDAC that had zero board certified ER docs. they were all misfits and castaways essentially doing GMO. i was legitimately scared for our peds population going to the ER. that changed with time, and i never knew I'd be so happy to see a "regular" EM doc staffing an ER to see kids. peds EM at most locations is a luxury we probably don't really need. so i guess it's all a matter of perspective.

My next comment was that the best route to do peds EM would be either EM or peds as a civilian, but that's a whole different topic.

The simple answer is that it isn't going to happen. There are no pediatric emergency departments anywhere in the military, on any continent, at any base. You can be an emergency physician and see everything in the ED, or a pediatrician who works in the clinic.

The Navy actually trains quite a few of them. They have at least one for every major MTF and I think they're actually trying to increase that.

However they do only offer it to the Emergency Medicine guys. My specialty leader says they've never approved a Pediatrician. Which makes sense considering
1) There's a serious question of which specialty would own them after fellowship
AND
2) The ED views Peds -> PedsEM as a scheduling nightmare for the ER, since they basically couldn't take any of the solo overnight shifts, or even help out with a day shift that was heavy on adults.

I'm not sure its accurate that the Navy trains "quite a few." They need them for the residency program but there can't be more than 3-4 in the Navy total.

Army has a few people who have completed PEDS fellowship via the em route. They work more shifts in the ped pod than the other docs, but still spend a substantial amount of time I the adult section
