a 74 year old blind man is admitted with obstructive uropathy and chest pain. he has a history of hypertension and diabetes. his creatinine drops from 10/mg dl to 1.2 mg/dl three days after catherter placemetn.

why does creatinine drop in this contrast induced nephrotoxicity. ???

What contrast? The catheter is a Foley, not an IV catheter via which contrast was administered.

He has post-renal AKI, hence the high AKI with obstruction. Inserting the Foley voids the bladder and relieves the urine backlog, restoring kidney function and dropping the creatinine.
