Aural fullness when lying down?

A friend of mine is experiencing aural fullness, but only when she lies down. There is no associated vertigo or dizziness. She also reports that it only occurs in her left ear which she also feels has some hearing loss. She has not had any testing to document the hearing loss. Any ideas on what could be causing this?

sounds like a vascular pressure phenomenon to me for which there's multiple possible etiologies (glomus tumors, BIH, Otitis Media w/ effusion, eustachian tube dysfunction to name a few). How old is your friend? Any pulsatile tinnitus? Has she had anyone look in her ear? Is she overweight?

She should probably have a thorough otologic exam and probably and audiogram unless the cause is obvious.
