Bad grades on last rotation

Will grades or comments in rotations in the last semester of med school ever be seen by anyone? Clearly, everything is in for residency stuff- and they will probably just need a final transcript after graduation to make sure I graduated, but what about if I apply for fellowships? Do they see the C in the 4th yr elective? I'm not even sure where negative comments would go by this point anyway, bc deans letters went out long ago...
 
Doubt they would care even if they did see them. Unless you failed something required for graduation (or enough rotations that you couldn't graduate on time) they won't be able to change their mind if you match there.

Where do these comments even get recorded? Would any of this be on a fellowship application?
 
It might make it harder for you to get fellowship in this field ! But nothing is impossible in this life. Specially if you did an elective during your third year in the residency in the same field and corrected your negative experience.
 
If you fail something serious - like a rotation required for graduation (or, back in the day, CS) - then all bets are off. I personally know someone who butchered their post-ERAS M4 year so badly they had to withdraw from the match. If you get a C in an elective, no one will care.

You're right, the MSPE is already written, so I wouldn't worry about that. Some fellowships do require a medical school transcript as part of the application process, so they would see the grade (if they care to look that hard).

On the positive side, your performance in residency will determine ~99.8% of your fellowship options. And the period between ERAS and graduation is also the time in the curriculum where subpar performance is least likely to hurt you. That said, slacking off can always find some (previously inconceivable) way to come back and cause problems later on. I know two students who ran into serious trouble post-match and had their residency contracts voided before graduation. One is still in medicine.

TL;DR Keep it together and don't do anything stupid from here on.
