Based on my undergrad stats, will the path I have planned for Postbaccalaureate work help me get into medical school?

Because I used numerous life events as excuses to be distracted from school, I currently have a 2.94 cGPA and a 2.63 sGPA. I am graduating this May, and I am on track to get my cGPA to a 3.19 and my sGPA to a 2.96.
My plan is to return to my university after graduating, and spend a year to take about 25-30 credits in upper-level science courses. I believe I am figuring out which study methods help me to learn the most, therefore, I believe I can perform very well in my Postbacc courses. During this one year, I am planning on applying to 2 SMP's: Cooper Medical School at Rowan University and RWJ Master's in Biomedical Sciences. If I do well in one of these programs, will I have a chance at medical school?
