Hi all,

I'm in my neuro unit for class right now, and as always I study with Pathoma and First Aid for every block. I listened to the Pathoma for cerebrovascular disease, and am confused on a point Pathoma made about berry aneurysms.

In Pathoma he says that they commonly occur at branch points (usually the anterior CA - ant comm branch point, similar with posterior) because at the branch point the vessels lack a Tunica Media. This I completely understand, but I'm now confused as to whether the lack of media in branch points is found in all people, or just people who are predisposed to BA's (ADPCK, Marfans, etc.).

Goljan said it's found in all people. In his words, something like "we all have that defect; you all may be working on an aneurysm right now!" So, it's only the hypertension in ADPKD that makes them more predisposed; someone with poorly managed essential hypertension would theoretically have the exact same elevated risk.
