Best Dictation Errors in Pain

just noticed this am looking at a note from last fall - I dictated "I injected the piriformis" , it was typed "I injected the area for months"
 
from my co-resident's note:

"the patient also has persistent priapism that he cannot control"

(I think she meant he has persistent spasms that he cannot control)
 
Will consider a trial of "highly ironic" acid injections for his bilateral knee osteoarthritis.
 
...the patient displays decreased "gay" tolerance with short step length on the left......


...no evidence of "eggs you hate" (exudate)........

On a patient whose insurance changed and she had to switch pain doctors (from a colleague of mine to my practice):

Dictated: "All that I can add to Dr. X's previous care is a little embellishment".

Transcribed: "All that I can add to Dr. X's previous care is a little intelligence"

. . . and cc'd to the other pain doctor.
 
Patient I had recommended for lumbar facet RF ablation.

"I feel that the patient would be a good candidate for lumbar annihilation".
 
Transcribed: "The L4-5 disc protrusion indents the fecal sac."

Dictated: "The L4-5 disc protrusion indents the thecal sac."
