Cool. How was your overall experience doing that fellowship. Was it like a service provision job similar to Uk. How do you compare it to Uk training. Did you find it harder?

The fellowship helped me understand the logistics of the system and refined my clinical skills. It also made me way more marketable to US employers. ACGME structures training programs in the US in a very specific way just like core training in the UK, or maybe even better.

I hope you're doing well. I have a few questions and would really appreciate your help:

I already sent emails to several states, including North Carolina, Oklahoma, Michigan, and Texas. The only state that is accepting MRCGP with ECFMG and USMLE (all steps) is Maine. I am really confused about this, and the only logic I can come up with is that there must be a sequence to this process. I am not doing well financially, so I am thinking of doing this on my own without involving any agency. Furthermore, I would really appreciate it if you could guide me as much as possible.

1. Do I need to complete a fellowship after obtaining my Maine state licence to apply for licences in other states? Can you shed more light on this? Specifically, what was the sequence in which you applied for other state licences? I want to apply for a Michigan or Georgia licence after getting a full licence from Maine.
2. I have an MRCGP in Family Medicine, and I am looking for a one-year fellowship in pain medicine. I checked the FREIDA AMA Residency & Fellowship database, but Pain Medicine it’s not offered in Maine this year. Do you know of any other databases for fellowship programs?
3. Did you apply for a Compact Licence after completing your fellowship? If yes, do we have to complete the fellowship from the same state where we get our first full licence to be eligible for the compact state agreement?
4. Can you provide an estimate of how much Road to USA charges for their packages? I find their services quite expensive, since they are charging $300 just for the first online appointment.

I did not apply for the compact and do not need it as of now. I am not sure if I even qualify.

The more you need them involved, the more you have to pay. I ended up paying them thousands of dollars. However, now I easily make the total amount paid in a month or less of work. Totally worth it in my opinion; I spared myself messing things up in the middle and losing it all. I suggest you shop around. Maybe you can find a more affordable competitor?

And thank you very much for all your responses so far, I have come up with few new questions and I think that you are the best person to answer these, so I really appreciate if you can give me a little bit more guideline.

1) I saw in your previous posts that you are doing geriatrics fellowship, I would like to know, do you need to do home visits as well during and after you complete your fellowship.

2) Do you have any idea about how easy it is to get fellowship in pain medicine for someone who is coming from a family medicine background in US?

3) Can you recommend few specialties that have good income potential for opening up a private clinic.

4) Further from your point of view how competitive it is to get fellowships in USA? What's your personal experience on this how many specialties you applied for? Where you able to secure few spots on first attempt?

#1 No home visits since I finished fellowship
#2 I have on idea
#3 & #4 are complicated questions and I do not have time to write a lengthy response. Basically, the consultant that I hired guided me to make the best choice that fits my needs and expectations and I am fairly satisfied with the outcome so far.
