How difficult for out of staters?

Better to try for the private schools in California (Loma Linda, USC and Stanford), and not bother with the publics unless you're applying MD/Ph.D. Although I think UCSF admits a fair fraction from out-of-state.
 
UCSF and UCLA are actually fine for out of staters with the most amazing stats out there. They take over 40% out of state each year. Of course, regardless of your residency you have to be among the absolute tops to get into either. Irvine and Davis basically do not take out of state students. SD takes a few out of staters, but nowhere near the amount LA and SF do. This is an extremely tough state to get into or stay in, make no mistake about that.
