This is a violation.

Who in their right minds would have noticed this on a day to day basis? Hats off to you if you do every and all the time! I think the ones who printed this should be getting the fine, not us.

Yeah why should pharmacy get fine rather than the provider who uses these.

I actually would but I agree that prescribers and security printers should get fined.

If California DOJ authorized only one or two types of forms (one Rx per pad and/or the multi-Rx pad) it would make life a lot easier. It is annoying to explain to ******ed office managers or MAs why security printer ID and preprinted DEA and address are requirements
