Can I include this in my app?

I volunteered at Vincent de Paul, which is an organization that help prepare and serve food to homeless people.
But It's not a routine work; it's more like come in whenever you feel like it, and I only did about 4 times over the summer for 3-4 hours each time.
I was wondering if it's ok to put that in the app or is it too insignificant?

I wouldn't bother, unless you're real short of volunteer activities. If that's one of your more notable ones, you might want to try to work on that.

I think it won't hurt to list it.

list it. i think med schools want to see how even after getting your mcats back and being bogged down wiht apps you still went out and did something for the community. LIST IT!
