My understanding was

Primary->dormant->secondary when immunocompromised->apical focus, disseminated disease etc.

But first aid it seems like primary and secondary are different? Can someone help me out here?

You might need to elaborate your question better. What your understanding is sounds right, but your last statement doesn't seem to conflict with that, unless I'm misreading

In 1st aid it seems like secondary arises from its own after the person was exposed to an organism before with no colonization. That's what it seems like to me

Ah ok. WIthout opening up FA, your understanding is correct regardless of whatever FA says. The apices involvement is classically due to secondary infection, which arises from a primary infection that goes dormant within a ghon focus
