The first polar body forms in the ovary.

The second and third polar bodies form in the oviduct.

I'm not sure (although I'm sure Google knows if you try it), but I can tell you this--I HIGHLY doubt the DAT would ask you anything this specific. The DAT wants to make sure you know a little bit about everything. "Breadth over depth" is the motto for studying for the DAT sciences.
