Can you moonlight internally on an H-1B visa with an unrestricted medical license? It would be at the same institution, picking up extra shifts for extra money. I'm applying for an unrestricted medical license now, and I don't see why it wouldn't be possible?

Theoretically, yes.

Practically, the institution might require a second H visa (or an update to the terms of your visa) and may either decline to do so, or simply not want to take any risk with the situation.
