Cant find "Print the Letter of Evaluation Matching Form"

I cannot find "download" the PDF version of the Letter of Evaluation Matching Form.

I could not find the matching form although I already followed their instruction. I could not find the download tab for this form in evaluators area. If anyone can find it, please help me

I realized that if you type information and save it,
it would have the print form afterwards

But it say the LOE is pdf form

1. Login to your AADSAS application
2. Select Evaluators from the Application Checklist
3. Download and print the PDF version of the Letter of Evaluation Matching Form
4. Submit the form to each evaluator. Be sure to advise the evaluator to enclose the form with the letter and mail it directly to AADSAS.

Does anyone download this LOE pdf form??

Did you type all the info and "SAVE"?
After you save it, on top left side corner, you will see "print matching form"
when u click it, pdf file pops up.

It worked fine for me. You should save your info and look for a little icon on the page where it lists your schools.

You have to choose if your evaluator is submitting electronically or by mail. If they are doing it electronically, then you'll enter their email address and there will be no matching form to print out (AADSAS will email them directly and give them instructions). If they are submitting by paper (through the mail) then you have to check that bubble and you will not fill in their email address. After you save their information, there will be the option to print out the LOE matching form. Hope this helps!
