Caring for a hospice patient in the community

I have the opportunity to care for patient who is currently on hospice. By caring, I mean spending time with, preparing meals, shopping for groceries for the spouse who lives with the patient etc. It came to my attention through word of mouth, not really through any specific organization. How does one go about documenting this type of volunteering.
 
Not being paid. This would be a volunteering position for a few hours per week. It is for an elderly man that is a friend of my parents.
 
You would list the dates, the hours, maybe list the spouse and her contact info as the person who could confirm. You'd call this volunteer NON-CLINICAL given that the role is not in a health care facility and you aren't doing anything for the patient specifically but helping with what we call instrumental activities of daily living.

It is a blessed thing to accompany a family at the end of life, even if it doesn't count for much in the larger scheme of med admissions and I'm sure you'd do it anyway.
