So I have, among my specific specialty LORs, a LOR from Internal Medicine that I may or may not choose to send for my specialty application for ERAS. I plan on using it for my prelim application, but I'm also currently doing a medicine sub-internship and hoping to get a better LOR from that this month. If I am somehow unable to do so, I would send my original (but perhaps not particularly exciting/compelling) IM letter along with both Prelim and Derm applications, but if I can, I'd rather get an IM sub-I letter and send that to Derm programs (along with 3 Derm letters).

If I assign my 3rd year clerkship IM LOR to Prelim/Derm right now in ERAS (for my school's uploading purposes), but then change my mind in a few weeks (after already submitting my application), I guess I couldn't "remove" this LOR from my Derm application at that point, assuming programs have started to download my application, would I?

Hopefully you followed all that.

Your question is answered in this thread.

Short answer, go ahead and upload but don't assign yet. You can have 100 letters uploaded in ERAS that never get sent. But once you assign a letter to a program, you're stuck with it.
