changing to Neurosurgery late?

Is it possible to switch to neurosurgery at this late stage of the game (I'm a new 4th year), or is it too late since it's an early match? Secondly, if one matches in a surgical subspeciality that requires an intern year of general surgery, can one then switch subspecialties and enter the new specialty as a PGY2 (not having to repeat the intern year)?
