I still have 10 days of medicine wards, then Medicine shelf + Neuro shelf. And I thought last year was my most deserved Christmas break ever...
 
really smart to tell them (class of 2014) there is a competition.

Medicine shelf studying is getting old. If I continue my current pace, I will have finished MKSAP in one week. Plus 90% of Uworld questions. This winter break will be amazing...

Last call night on wards tomorrow, woot.
 
Last day of medicine wards tomorrow! Happy for winter break soon, dreading the medicine and neuro shelf exams. Also kind of sucks knowing that next semester I won't have any rotation that I will like as much as medicine.

Surgery has been so much fun. I'm bummed I won't get to do anything surgical-ish again until March, doing my ENT elective then. Yay!
Tomorrow is my last day on this rotation too.

On a different note, is anyone else having patients constantly attempt to set you up with their sons/nephews/some doctor they kind of know that's single? Is 25 some kind of magic number beyond which age you turn into an un-dateable pumpkin? Last time it happened I kept thinking "Lady, your colon is trying to pass into the great beyond as we speak and you are trying to convince me to go on a date with your friend's son?" Just weird.:laugh:

That's really weird.

I had my first real random medical advice experience the other day. I was at a convenience store buying some beer and some lady asked me about what to do for her kidney stones. I need to change out of scrubs ASAP.

I'm not a kid person either. For the little ones just try to ignore the crying/screaming. Realize that half the job (esp. for outpatient) is reassuring the goofball parents. Tons of asthma, URIs and diarrhea/dehydration (probably flu too at this time of year, prepare to get sick 🙁).

immunization stuff and the 4/2/1 rule seemed to be popular trivia here.

4/2/1 rule for maintenance fluids:
for first 10kg: 4ml/kg/hr
10-20kg: 2ml/kg/hr
above 20kg 1ml/kg/hr

some people do 100/50/20 for fluids over 24hrs (100ml/kg first 10kg, 50ml/kg 2nd 10kg, 10ml/kg for the rest and then divide by 24 for hourly rate).

The shelf I took was big on the different febrile rashes and child development stuff.

My heart goes out to you guys, kids are annoying to say the least.

I second this question - I start peds on Tuesday.

I saved peds for my last core rotation, just dreading the thought of doing it. I don't know what bothers you guys about it, but for me it is definitely the fact that kids just don't understand the concept of "little pain now saves a ton of pain later" (giving shots, cleaning wounds and the like). They will fight you like you are the devil incarnate sometimes. Obviously there are exceptions but the few experiences I had on Family Medicine has made me wary of dealing with kids.
Ah well, I have 4 more months until I can't avoid it. Internal now and as luck would have it, my team is on call on our first day on the wards. Yay! **sarcasmmmmmm**
 
Day 2 of Peds and I'm not a huge fan. I don't hate it, like surgery, but I can definitely tell I'm a medicine not peds person. I can see how people get into it, but it's not for me. I don't even understand how people can do med/peds, it just makes no sense.
 
I don't eat, I don't sleep, I feel like an idiot 24/7, and I'm loving every minute of it.
 
Had Medicine call today. So goddamn boring.

Bored so joining the fun. T-minus 4 months of third year left.

Thoughts for the peds question. Already did my peds rotation and thoroughly enjoyed it. I absolutely love watching master pediatricians interact with kid and parent. They are simultaneously playing with kid, talking with parent, and getting physical exam done. Amazing. I think one trick with kids is to not take yourself super serious around them. Be goofy if it suits you. And go slow. Don't just attack immediately and go for the heart exam. Parents trust you more if they see that you're gentle yet proficient. If you're not familiar with handling infants, they seem fragile. But most are pretty hardy ( - the preemies). Just don't drop them...lol

I am doing ENT elective in March too. Pumped.
I've been weaseling my way into ENT OR since May, no shame. Got to see a freaking neonatal ENT surgery, a microvascular flap procedure and helped wire a broken jaw (apparently UH is the only place that does that even around here, bloody mess at 4 AM).
I can't wait to actually be in the OR in any kind of semi official capacity lol.

Never really considered it, too many away rotation kids that come are wayyyy too gunner, it seems to be the culture during application year. What's it like at your institutions?

From my experience with residents/students at 2 dif institutions, as well as my own experience (I've known I wanted to ENT since senior year of college) this is my summation:
ENT is a field of slightly dorky kids who are just naturally more curious than average. You get personality variations, but the general trend for me seems to be people who are pretty relaxed but get so damn excited about learning and ****.
I am sure there are some gunners who end up in ENT out there but I wouldn't describe any person I've met who is doing or interested in ENT as one. From personal experience I can tell you that knowing that you have such a high bar set can make you really anxious about basically having to be "perfect" to be a good candidate. I mean average Step 1 score is 240+, jeezzus. My neuroses have been having a field day since May with all this lol.
ENT is an unusual surgical specialty because it is clinic heavy and actually pretty Medicine-heavy which people quickly equate to fabulous lifestyle. But there are also routine surgeries that last 7+ hours long. I don't see a good gunner liking that at all.

I know a couple of people who are interested in ent that are gunnerish. The few others are chill. All of the residents and Attendings I've interacted with have been laid back, intelligent, and engaging, which is more or less the stereotype for the field.

Personally, I like the fact that in such a confined space as the head and neck there's a wide range of variability you can do. Plus not to mention the good mix of medicine and surgery. Plus what tat said, lol.
 
Ya I've had great experience w/ ENT residents, just the 4th years that rotate, obviously need to impress, and probably act more gunner than usual. It just seems so high stress (To get into)
 
Med prom was so much fun. Glad im finally done with the sads.
Internal medicine, however, is wearing my patience thin.
 
My patient has Pulmonary Langerhans Cell Histiocytosis...that was quite a shocking CT scan to start the morning off with.

Started family medicine orientation today. Get my real start on Wednesday. Hopefully won't have horrible work hours. Shelf in four weeks, but not starting studying until after Mardi Gras. So from now until then it's parades and reading/trying to get some case reports done.
 
You people make me sick.

Still loving surgery. Very tired though. And I'm pretty sure I haven't eaten a fresh vegetable in more than a month.
 
What do y'all think about only giving myself ~2 weeks after third year ends to study and take Step 2 CK? I really just want to get it over with, especially if I can start doing aways in July.

But a lot of places don't want visiting students in July. I don't really want to waste the month by taking it off to study, but I might have to.

i am takin july off to take CK and CS and rest up for my aways. i will have finished my last core at end of june too, so its perfect timing.
alot of ppl do their home sub-is in july so i think ud be fighting a losing battle tryin to get an away then but i could be wrong about that.
