Clinical Hours Real vs Fake Name Question

I work as an in-home caregiver for a man, and this person wrote me a letter of recommendation this cycle (he does this pretty commonly). During my personal statement and work and activities, should I use his real name because ADCOMs will see his full name in my LOR and as a contact for my work/activities section. Just don't want someone who doesn't have the full context to be thrown off or confuse people if I use a fake name when they have the real name. Any advice is appreciated. Thanks!

That's my understanding. Frankly, I can't recall ever seeing such a letter -- maybe once-- the person was a working professional with a disability who had several people who took shifts helping with activities of daily living (bathing, dressing, toileting, etc). Or I might be imagining that such a person would write a letter.

I would rather not have the letter from this man and have you use a pseudonym than to have you jump through the hoops of using his real name, running the essay by him and then adding a footnote stating that you are grateful to him for permitting you to use his name.

yes, it’s a caregiving system that basically makes the person receiving care their own boss to give them more autonomy in their care. They are in charge of hiring, advertising, ordering supplies, etc.
