Working in an academic medical center, particularly one with or near a strong neuroscience department, could increase your chances of doing the type of research it sounds like you want to do. As you've mentioned, research projects and slants can differ significantly from one setting to the next, and even from one department to the next in the same setting.

Given your interests in practice, it seems as though neuropsych would still be the best psychology-related field for your current goals.
